After booking your free discovery call, you will receive an email with a short intake form. This helps us better understand your case, your health history, your goals, and the type of care you are seeking. Our team will review your intake form before the discovery call. If, after review and conversation, your needs align with our scope of care, we may invite you to schedule a new patient visit to establish care and discuss your case in depth. Established patients do not need to repeat this step — you may book the appropriate follow-up or ongoing-care visit type directly.

If appropriate, we will invite you to schedule a new patient visit to establish care and discuss your case in depth — including your history, goals, treatment planning, and program selection if applicable.
